Aptos thread otoplasty uses the Aptos Sole Otoplasty N thread system, a specialised non-absorbable barbed thread developed specifically for ear cartilage reshaping. The barbs grip the cartilage tissue and hold it in the corrected position permanently, without the need to cut, remove or surgically suture the cartilage as traditional otoplasty requires.
The procedure is performed entirely under local anaesthetic in Dr Barker's rooms. A double-pointed needle is used to insert the thread through a single small entry point behind the ear. The thread is passed through the cartilage at the precise angle and depth required to produce the correction, and once set, the barbs engage and hold the new position immediately. The entry point is minimal, heals cleanly and leaves no visible scar in the vast majority of cases.
Results are visible immediately after the procedure. The ears are repositioned as soon as the threads are in place, and patients can see the correction in the mirror before leaving the room. Mild swelling and sensitivity around the ear resolve within 1 to 2 weeks. A soft headband worn for 7 to 10 days supports the position while the surrounding tissue stabilises around the threads.
Published studies report an 87 to 90% rate of exceptional or very good outcomes at one-year follow-up, with no reported cases of infection or haematoma. While the result may not be as permanent as conventional cartilage surgery in every case, the vast majority of patients maintain their correction long term, and a touch-up procedure is straightforward if needed.

Dr Barker assesses the ears, the degree of prominence or asymmetry and confirms suitability for the Aptos thread approach. He will be honest if surgical referral would be more appropriate for a particular presentation. The degree of projection, cartilage flexibility and the patient's goals are all considered before recommending treatment. The consultation is complimentary.

Local anaesthetic administration
Local anaesthetic is administered around the ear to ensure the procedure is completely comfortable. This takes a few minutes to take full effect. Once numb, the procedure itself is painless. Most patients report feeling pressure or movement rather than any discomfort during the thread insertion.

Thread insertion and correction
Using the Aptos double-pointed needle, Dr Barker passes the barbed thread through the cartilage via a single tiny entry point behind the ear. The thread is placed at the precise angle to achieve the desired correction. Once the thread engages the cartilage and the position is confirmed, the same process is repeated on the second ear to achieve symmetry.

Immediate review and aftercare instructions
Once both ears are treated, Dr Barker reviews the symmetry and position with the patient. Results are immediately visible. Aftercare instructions are provided, including guidance on the headband, sleeping position, hair washing and activity restrictions. The entry points are so small that no dressings or bandages are required in most cases.

Recovery and follow-up
Mild swelling, redness and sensitivity around the ears resolve within 1 to 2 weeks. A soft headband is worn for 7 to 10 days to support the corrected position. Contact sports and activities that could impact the ears should be avoided for 3 to 4 weeks. Dr Barker follows up to confirm the result and address any questions.
