What makes the Fotona 4D Facelift unique is its four-dimensional approach. Most laser treatments work only on the surface of the skin. Fotona 4D begins inside the mouth, heating the submucosal tissue to stimulate deep structural collagen, then works outward through increasingly superficial layers of the face. The result is a lifting and tightening effect that feels natural because it is generated by your own biology.
Dr Barker personally performs every Fotona 4D treatment. The protocol requires a thorough understanding of facial anatomy and laser physics, and the outcome is directly dependent on the skill and experience of the practitioner. This is not a treatment to be delegated to a therapist.

The Four Modes
How the 4D protocol works
Each of the four laser modes targets a different depth and tissue type. Together they address every dimension of facial ageing in a single session.
The treatment begins inside the mouth. A non-ablative Er:YAG laser handpiece heats the mucosal tissue from the inside, stimulating deep collagen production in the cheeks, nasolabial folds and jowl area. This is the unique step that sets Fotona 4D apart from all other laser treatments, delivering a lifting effect from the deepest layer outward.
A fractional Nd:YAG laser targets deeper skin imperfections including pigmentation, vascular lesions and collagen depletion in the dermis. This step addresses structural damage beneath the surface, improving overall skin quality and tone in ways that topical products simply cannot reach.
A bulk heating mode that delivers controlled thermal energy into the deeper dermal and subcutaneous layers. This produces significant tissue tightening and further collagen remodelling. The skin is heated evenly and safely, creating a sustained lifting effect that continues to develop over the weeks following treatment.
The final step resurfaces the outermost layer of skin using a light Er:YAG pass. This smooths texture, refines pores, evens tone and creates the immediate glow that patients notice straight after treatment. In the advanced version this pass is intensified to produce a deeper peel and more significant surface renewal.
What to Expect
Your treatment, step by step

Consultation
Dr Barker assesses your skin, discusses your goals and recommends which version of the treatment is most appropriate for you. He will explain exactly what to expect in terms of results and any downtime.

Preparation
The skin is thoroughly cleansed. Eye protection is provided. For the intraoral SMOOTH,er step, a small handpiece is placed inside the mouth, which most patients find comfortable and easy to tolerate.

The Treatment
Dr Barker works through all four laser modes in sequence. The full treatment takes approximately 60 to 90 minutes. Most patients describe a warming sensation. The intraoral step is well tolerated and takes around 15 minutes.

Immediately After
For the standard version, skin will appear flushed and glowing. Redness settles within a few hours. For the advanced version, more significant redness and the beginning of a peel is expected, with full peeling occurring over days 2 to 5.

Results and Improvement
An immediate improvement in tone and firmness is visible after both versions. However, the deeper collagen remodelling continues for up to 3 months, with results continuing to improve progressively. Most patients achieve optimal results with a course of 2 to 3 treatments spaced 4 to 6 weeks apart.
